New patients will need to complete a brief patient registration form. Please print it out, complete it, and bring it with you to your appointment.
You will note that there are three signature lines: you will be offered the HIPPA compliance policy at the first visit. You can take it or leave it, but it is the law that you are offered a copy. You can sign at that time if you would like.
Be sure to bring your insurance cards and your prescription from your physician if you have it.
